I view South Africa as a country wide version of an American big City. Complete with black rule and lack of law enforcement. I also don't know why any white people stay, but probably because they either live in a white enclave or can't sell their property for anything.

Only from detonation above a certain amount. I think it's 220 kilotons. Airbursts vs groundbursts matter, too. Everything below that size of an exlosion is mostly local fallout.
